Why does the US have 20% of total COVID19 deaths while having only 4% of the world's population?

Questions:

1) Are they categorizing all the deaths incorrectly?
2) Is the population very unhealthy?
3) What's the effective IFR in the US?
4) Have there been updated serology tests in the US? I recall during Spring more than a quarter of New Yorkers had COVID antibodies
5) What's the average age of people dying with COVID in the US? I know in the UK it was over 80 (in-line with life expectancy)
